Physician response was in reaction to the congressional action taken this past June to avert the Medicare payment cut. It was ultimately delayed and increased to approximately 23% effective on December 1, 2010. In addition, another reduction of 6.1% was added to the Medicare Part B conversion factor effective January 1, 2011.

Sustainable Growth Rate, 2010

The new report indicated that of the over 63,000 physicians who responded, they felt that cuts in Medicare reimbursement would result in cuts in their practices as well.

Overall, the survey found that while approximately 92% of practices are currently accepting Medicare patients, this number could change drastically. And the reimbursement cuts would likely result in:

- Limiting the addition of new Medicare patients (67.2%)
- Reduction in the number of administrative support staff (60.5%)
- Reduction in the number of clinical staff (54.0%)
- Delayed purchase of electronic health systems (45.3%)
- Delayed purchase of electronic prescribing technologies (42.4%)

According to the survey, results align with decisions undertaken by physician practices after Congress postponed action on reimbursement cuts in June. Unfortunately, even though over three-quarters of U.S. physicians are dissatisfied with Medicare reimbursements, there is little consensus on how to reform the system.

A secondary national survey of approximately 6,000 randomly selected physicians, rated physician support for several Medicare reimbursement reform proposals.

Of the 1,222 physicians who responded—most of whom accepted Medicare—approximately 78% thought that some Medicare procedures were either too highly compensated or not compensated enough.

Nearly half of respondents favored offering incentives—such as bonuses for achieving certain quality standards and preventing re-hospitalization—as a means to payment reform. However, 46% of physicians did not support shifting payments away from procedures and toward management and counseling services and 69% of respondents said they opposed bundling payments.

In the end successful implementation of payment reforms may hinge on understanding physician concerns with proposals and their willingness to compromise or make tradeoffs.

Visalia, CA -- (SBWIRE) -- 12/22/2010 -- In 2005 The Center for Medicare and Medicaid Services (CMS) launched a pilot project geared to measure the success of group physician practices. Five years later the results are mixed, as practices have increased quality but have failed to reduce costs as much as predicted.

Medicare Pilot Savings Project

Over the past five years CMS has run a pay-for-performance, shared savings demonstration project with ten physician groups. The goal was to test two of the basic beliefs behind health care reform: that raising quality lowers costs and that group practices offer the best vehicle for weaning physicians from fee-for-service medicine.

The project measured the physician groups' adherence to 32 different procedures which had previously proved successful in lowering costs and improving patient outcomes. These included health care procedures such as routine eye and foot exams for diabetics, administering beta blockers during congestive heart failure hospitalizations and ensuring patient compliance with medication schedules for blood pressure and cholesterol control.

In addition, the CMS provided funds to offset some of the start-up costs. These included paying for nurse practitioners to monitor Medicare patients while encouraging them to take their medications. If the group practices steadily improved their performance each year while keeping costs from rising over other Medicare providers in their region, they received 80 percent of Medicare's savings.

By 2008 CMS found that every physician group involved had achieved high levels of performance and achieved benchmarks on 28 of the 32 quality measures. In addition, five groups had reduced costs.

Debate Continues Over Pilot Projects

While the Government Accountability Office (GAO) noted that the Medicare program showed promise, the Congressional Budget Office (CBO) projected only a $1.3 billion in savings from all the demonstration projects recommended in the bill. However, the CBO admitted there was no real way of determining the outcome since several of the programs weren't yet designed or they had no track record.

The question now is whether the CMS will be able to extend the positive outcomes from the project to the wider medical community. "The largest majority of physician practices in the country," notes Weinstock, "have fewer than 200 physicians. These smaller practices may have more difficulty absorbing the start-up and operating costs of the programs involved."

The overall goal is to move Medicare away from the fee-for-service model. Unfortunately, to have any kind of positive impact on Medicare requires much faster action.

Goal and Focus of HHS Study

The goal of the HHS study was to estimate the national incidence of adverse events for hospitalized Medicare beneficiaries, assess the preventability of such events and to estimate associated costs to Medicare.

For this report the term "adverse event" described harm to a patient as a result of medical care, such as infection associated with use of a catheter, bed sores and excessive bleeding due to blood thinners. "Never events" refer to a specific list of serious events, such as surgery on the wrong patient. In this case, surgery on the wrong patient accounted for less than one percent of these events. "Temporary harm events" are those that require intervention but do not cause lasting harm.

The physician review group participated in a four-step process. They determined whether an adverse event occurred, whether it was a reportable event based on the National Quality Forum (NQF) list of Serious Reportable Events or the Medicare list of hospital-acquired conditions (HAC), the level of harm to the patient and if it was deemed preventable.

The study drew on a nationally representative sample of 780 Medicare patients discharged from hospitals in October 2008.

Overview of Outcomes from HHS Study

In addition to the 13.5 percent of hospitalized Medicare beneficiaries who experienced adverse events, the HHS study found:

- An estimated 1.5 percent of Medicare beneficiaries experienced an event that contributed to their death, which projects to 15,000 patients in a single month.
- An estimated 13.5 percent of Medicare beneficiaries experienced events during their hospital stays that resulted in temporary harm.
- Approximately 28 percent of beneficiaries who experienced adverse events also had temporary harm events during the same stay.
- That 44 percent of adverse and temporary harm events were clearly or likely preventable.
- That 44 percent of all events were preventable and 51 percent were not preventable.
