Old Hook Medical - Patients Centered Medical Home

Old Hook Medical focuses on putting patients first through its alliances with organizations of International repute which help implementing a more cost effective and patient focused system in place.

Emerson, NJ -- (SBWIRE) -- 04/18/2012 -- When primary care physicians are responsible for coordinating patient care and healthcare services, good things happen. New Jersey is once again in the forefront of positive changes in the healthcare delivery system. Through its Patient-Centered Medical Home, Old Hook Medical Associates (OHM) is taking part in a pilot program with Horizon Blue Cross/Blue Shield that has already reduced Emergency Department visits by 26% and lowered medical costs by 10%. The program has also led to better diabetes control among patients and a higher level of patient compliance with screening tests. Horizon states that the program has already been so successful that it will be expanded to approximately 200,000 patients by the end of 2012.

Many medical practices have made major investments in information technology to implement Electronic Health Records that can be shared by all physicians in the group. Today's well-informed patients also seek practices withan online Patient Portal where patients can securely access their records 24/7 from any computer.

OHM continues to put patients first through joining the Hackensack Physician-Hospital Alliance based at Hackensack University Medical Center. This alliance is one of only three Accountable Care Organizations approved by Medicare in New Jersey. This collaboration relies on the group's ability to process patient data such as preventive care measures such as flu shots and cancer screenings and helps identify high-risk patients in need of more careful monitoring. The program complies with the spirit of health care reform which will migrate physician payment away from fee-for-service and into a system which is more cost effective and patient focused.

Patient Centered Medical Home Benefits Patients

RTI conducted a survey of Patient Centered Medical Homes (PCMH ) and analysed that facilities provided by PCMH benifits Patients. Old Hook Medical is pleased with survey which reflects quality care provided by their practice and qualified at the third and highest level for NCQA recognition as a PCHM.

Emerson, NJ -- (SBWIRE) -- 10/21/2011 -- The results of a recent study, conducted by RTI International, a nonprofit research organization, analyzed patterns of care, health outcomes, and costs of care for Medicare fee-for-service (FFS) beneficiaries. The study compared the results from practices that are National Committee for Quality Assurance (NCQA)-recognized medical homes with those that are not, seeking to determine if there are particular attributes of medical homes that are more favorably related to better outcomes of care.

The study showed that patients of Patient Centered Medical Homes (PCMH) have a lower hospitalization rate, better follow-up care after hospitalization and a lower hospital readmission rate than patients at other facilities. In addition, total Medicare payments per beneficiary were 43% less to physicians associated with PCMHs.

Old Hook Medical Associates, a PCMH in Emerson NJ, scored favorably in many target areas, compared to both its benchmark and other PCMHs. "We're pleased that the results of the survey reflect the quality care provided by our practice", said Edward Gold, MD, a partner in Old Hook Medical Associates. "It reinforces our belief that PCMHs lead to better health care quality, improved involvement of patients in their own care, and a reduction of costs."

Through the Patient-Centered Medical Home at Old Hook Medical, each patient develops an ongoing relationship with a personal physician who leads a team providing for all healthcare and when needed, arranges for appropriate care with other qualified physicians. A PCMH emphasizes enhanced care through open scheduling, expanded hours and strong communication between patients, physicians, and staff.

Old Hook Medical is the only practice in Bergen County to have qualified at the third and highest level for NCQA recognition as a Patient-Centered Medical Home. In addition, the practice is already certified by the NCQA in Diabetes, and Heart/Stroke Care.

For more information on this press release visit: http://www.releasewire.com/press-releases/release-3.htm

Media Relations Contact

Edward Gold
Old Hook Medical
Telephone: 201-666-3900
Email: Click to Email Edward Gold
Web: http://www.oldhookmedical.com/
